体外膈肌起搏治疗病毒性肺炎的研究综述
摘要
来展望及争议考量等方面。体外膈肌起搏技术在改善病毒性肺炎患者膈肌功能与肺功能上展现出一定潜力,但在安全性、
伦理及成本效益等多方面仍需深入研究。文中详细阐述了该技术的历史演变、作用机制,病毒性肺炎的流行病学特征与病
理生理机制,以及体外膈肌起搏在临床应用中的治疗方案、效果、安全性评估等,并探讨了其未来发展方向与面临的挑战,
旨在为相关领域的进一步研究与临床实践提供全面参考。病毒性肺炎在全球范围内对各年龄段人群均有影响,尤其在儿童
和老年人中负担较重。在社区获得性肺炎中,病毒作为病原体的占比日益增加。一项对 488 例老年社区获得性肺炎患者的
研究发现,感染病因明确的患者中,病毒感染占 8.4%,常见病毒为甲型流感病毒和呼吸道合胞病毒 [6]。不同年龄段感染的
病毒类型存在差异,如足月婴儿以呼吸道合胞病毒为主,早产儿则以副流感病毒 3 型为主 [7]。免疫状态也影响病毒感染类型,
免疫功能低下患者在流感季节,流感病毒感染增多,非流感季节则以巨细胞病毒为主;而免疫功能正常患者在非流感季节,
呼吸道合胞病毒、腺病毒等感染更为常见 [8]。病毒性肺炎患者住院死亡率较高,如一项观察性队列研究中,284 例病毒性肺
炎成年患者的住院死亡率达 23.2%[9]。病毒性肺炎的病理生理机制复杂,病毒入侵呼吸道上皮细胞后,引发免疫反应与炎症
级联反应。病毒感染导致肺泡上皮和毛细血管内皮受损,破坏上皮 - 内皮屏障,引发急性呼吸窘迫综合征(ARDS)。如
新型冠状病毒可通过与血管紧张素转化酶 2(ACE2)结合进入细胞,导致肺部炎症和细胞死亡 [10]。病毒感染还会激活免疫
细胞,释放炎症因子,如肿瘤坏死因子 -α、白细胞介素 -1β 和白细胞介素 -6 等,进一步加重炎症反应与组织损伤。研
究显示,在 poly(I:C)诱导的大鼠病毒性肺炎模型中,清瘟解毒合剂可下调这些炎症因子表达,减轻肺泡 - 毛细血管屏障
破坏 [11]。此外,病毒感染可能影响肺部代谢过程,如精氨酸代谢、花生四烯酸代谢等,进而影响肺功能。病毒性肺炎会损
害膈肌功能,影响患者呼吸。流感病毒肺炎可导致肺上皮严重损伤与呼吸衰竭,影响肺干细胞及其再生反应,进而间接影
响膈肌功能[12]。新冠病毒感染患者康复后,肺功能常出现限制性通气障碍和轻度弥散功能缺陷,这可能与膈肌功能受损相关。
一项对 50 例新冠病毒肺炎康复患者的研究发现,与对照组相比,患者的总肺容量、FVC、FEV1、FEV1/FEV 及一氧化碳弥
散量(DLCO)显著降低,50% 的患者出现限制性肺损伤,35% 的患者存在轻度弥散功能缺陷 [13]。此外,病毒性肺炎引发
的全身炎症反应可能累及膈肌,导致膈肌肌无力,增加呼吸做功,进一步加重呼吸负担。而体外膈肌技术,作为一项安全
有效的方法,对于治疗病毒性肺炎的呼吸功能障碍,有显著效果 [27]。
