qSOFA、NEWS评分及临床症状对急性高原性肺水肿的 早期诊断价值
摘要
AssessmentqSOFA和国家早期预警评分National Early Warning ScoreNEWS模型对急性高原性肺水肿highaltitude pulmonary edemaHAPE的早期诊断价值。方法这项单中心前瞻性研究招募了2021年06月至2022年06月就诊于拉萨市人民医院急诊科近一周从低海拔小于1000m至高海拔拉萨市3650m地区并出现高原反应相
关临床症状并分为急性高原性肺水肿组HAPE组和高原反应组非HAPE组。比较两组基线资料、qSOFA、
NEWS评分及临床症状将组间差异指标进行logistic回归分析寻找与HAPE发生相关的指标将相关指标进行
COX回归分析寻找可用于急性高原性肺水肿早期诊断价值的指标。结果采集了759名患者的临床数据包括病
史、体格检查和辅助检查结果。其中100人诊断为高原性肺水肿患者461人为普通高原反应患者组间咳嗽、呼
吸困难、SpO2、qSOFA评分和NEWS评分均存在明显差异p<0.05COX曲线显示咳嗽、呼吸困难、qSOFA和
NEWS评分可以作为预测急性高原性肺水肿的指标。结论咳嗽、呼吸困难、SpO2、qSOFA评分和NEWS评分这5
个指标与急性高原性肺水肿的发生明显相关p<0.05。COX曲线显示咳嗽、呼吸困难以及结合qSOFA、NEWS评
分后的改良版模型可以作为预测急性高原性肺水肿的指标。
