复合基台设计对三单位种植固定桥疲劳强度及 修复螺丝稳定性影响的初步研究

摘要

目的 探究不同设计特点的复合基台在三单位种植固定桥应用中的疲劳强度差异,并分析在疲劳载荷作用下修复螺丝的变化情况,以期为临床上复合基台的设计和制作提供指导和建议。方法 本实验将一体式六方、一体式上部六方、分体式四方复合基台分为OHCA、OUHCA、TSCA三组。依据ISO 14801-2016,每组2个种植体埋入PMMA树脂,固定螺丝。样本固定在疲劳机上,测静态破坏载荷、疲劳极限载荷及弯矩。记录修复螺丝扭矩损失、断裂情况,并使用单因素方差分析比较三组间扭矩损失。结果 1.OHCA、OUHCA、TSCA三组的静态破坏载荷分别为2130N、1500N、1641N,疲劳极限载荷为500N、300N、450N,最大弯矩M为2750Nmm、1650Nmm、2475Nmm;2.三组在疲劳极限载荷下修复螺丝均扭矩损失,但差异无统计意义;3.OHCA和OUHCA组螺丝断裂主要在上部螺纹,TSCA组断裂可遍布螺丝全长,部分螺丝变形松动。结论 一体式六方复合基台具有更高的疲劳载荷承受能力,最大可承受500N,更适应口腔咀嚼力。然而,修复螺丝是各类基台的共同弱点,易在受力时折断,特别是六方和上部六方一体式基台的螺丝断裂位置相近。
